Obtaining Medical Records
 

All requests for medical records must be directed to Family Matters Counseling Center. Please email records request to familymatters@familymatterscounselingcenter.com.

 

A reasonable fee is included for providing these requested records as stated under the 22 Tex. Admin. Code § 165.2. The reasonable fee is a flat rate of $25 for pages less than 500 and $50 for pages over 500. These charges are included for labor, copying, and preparing the records. 

 

A copy of my medical records will be sent within 15 business days after payment is received for copies ("business days" do not include weekends).
