At Family Matters Counseling Center, we value strong collaboration with outside providers to ensure continuity, quality, and ethical care for shared clients. This page is designed for medical providers, mental health professionals, schools, case managers, attorneys, and community partners who wish to refer a client or coordinate care.

 

Make a Clinical Referral

If you would like to refer a client for counseling services, please complete our Referral Form below. Once submitted, our team will review the information and follow up as appropriate.

Use the referral form for:

• New client referrals

• Requests for counseling services

• Coordination of behavioral health care

• Questions related to scope of services or availability

For secure transmission of medical records or clinical documentation, you may fax records directly to:

• This fax number is for receiving medical records and clinical information only

• Do not use this number for general inquiries or outgoing correspondence

• All faxed documents are handled in compliance with HIPAA privacy standards

• Please include client name, DOB, and your contact information on all faxed materials
   

Care Coordination & Collaboration

We welcome collaboration with outside providers to support client treatment goals, continuity of care, and ethical coordination.

We commonly collaborate with:

• Primary care and specialty medical providers

• Psychiatrists and prescribing providers

• Schools and educational teams

• Case managers and social service agencies

• Legal and advocacy professionals
   

When appropriate releases of information are on file, our clinicians are happy to participate in care coordination.
